Vitex agnus-castus, commonly known as chaste tree berry, has been traditionally used for various gynecological conditions, including premenstrual syndrome (PMS). PMS Type C is characterized predominantly by "cravings," especially for sweets or carbohydrates, and often involves symptoms related to blood sugar fluctuations. The majority of clinical research on Vitex agnus-castus focuses on its efficacy for general PMS symptoms (such as mastalgia, irritability, mood swings, and headache), rather than specifically on Type C (craving) symptoms. Several randomized, placebo-controlled trials have shown Vitex agnus-castus extracts to be effective in reducing overall PMS symptoms, with proposed mechanisms involving modulation of prolactin secretion via dopaminergic pathways, and possible effects on other neurotransmitters or hormones. However, direct evidence for efficacy in specifically treating the craving or carbohydrate-related symptoms of PMS Type C is limited. Most studies do not separate out PMS subtypes in their analysis. Despite this, some reviews and guidelines (such as those from the National Institutes of Health and European Medicines Agency) recognize Vitex as a reasonable, evidence-based option for PMS symptom management in general. Thus, while traditional use and scientific studies support Vitex for PMS broadly, the evidence for its effect on PMS Type C (cravings) is moderate and indirect, leading to a rating of 3 out of 5.
