Vitamin B6 (pyridoxine 5-phosphate) has a scientifically established role in the maintenance of nerve health, and deficiency of vitamin B6 can cause peripheral neuropathy. Supplementation with B6 is justified and effective in cases where a deficiency is the underlying cause of neuropathy. There is strong evidence that restoring normal B6 levels reverses neuropathy symptoms in deficient individuals. However, there is limited or no evidence that B6 supplementation benefits individuals with peripheral neuropathy from other causes (such as diabetes, chemotherapy, or idiopathic neuropathy) if they are not B6 deficient. In fact, excessive intake of vitamin B6 (especially above 200 mg/day) can itself cause a sensory neuropathy, which underlines the importance of careful dosing. Several clinical reviews and case reports confirm both B6-deficiency neuropathy and B6-toxicity neuropathy, while randomized controlled trials evaluating B6 as a treatment for neuropathy in non-deficient populations are lacking or show little effect. Therefore, the routine use of vitamin B6 to treat peripheral neuropathy is only scientifically supported for documented deficiency states, and not as a general therapy for all forms of neuropathy.
