Synopsis
Propionyl-L-carnitine (PLC) is a derivative of L-carnitine and has been investigated for its potential benefit in peripheral neuropathy, particularly of diabetic and ischemic origin. The scientific basis for its use primarily comes from clinical trials and mechanistic studies.
Several randomized controlled trials (RCTs) have evaluated PLC in patients with peripheral neuropathy. For example, a double-blind, placebo-controlled trial published in Diabetes Care (2002) showed that PLC improved nerve conduction velocity and reduced neuropathic pain in patients with diabetic neuropathy. Additional studies in the early 2000s (Sima et al., 2005; De Grandis et al., 2002) reported similar findings, with significant improvements in neuropathic symptoms and some measures of nerve function.
The proposed mechanisms include enhancement of mitochondrial function, increased energy supply to nerves, improved microcirculation, and reduction of oxidative stress. PLC may also promote nerve fiber regeneration and repair.
Meta-analyses and systematic reviews generally support a moderate benefit for pain reduction and nerve function improvement in diabetic and ischemic neuropathies, though some reviews note methodological limitations and heterogeneity among studies.
Overall, while not universally accepted as standard therapy, there is moderate scientific evidence supporting the adjunctive use of Propionyl-L-carnitine in peripheral neuropathy, especially for symptom relief. It is considered relatively safe, but more large-scale, high-quality trials are needed to fully establish its efficacy and optimal use.
