Astragaloside IV is a saponin compound isolated from Astragalus membranaceus, a traditional Chinese medicinal herb. Its use in supporting or treating peripheral neuropathy has a basis in scientific investigation, albeit primarily in preclinical (animal or in vitro) studies. Several studies have demonstrated that Astragaloside IV can exert neuroprotective effects, including anti-inflammatory and antioxidant actions, which may counteract some of the pathophysiological processes involved in peripheral neuropathy.

For example, rodent models of diabetic neuropathy and nerve injury have shown that Astragaloside IV administration can reduce nerve damage, decrease inflammatory cytokine levels, and promote nerve regeneration. These effects are believed to occur through modulation of pathways such as NF-κB and Nrf2, both of which play roles in inflammation and oxidative stress. However, while these preclinical results are promising, there is a lack of robust clinical trials in humans to confirm efficacy or safety for peripheral neuropathy. The majority of the available evidence is therefore experimental, with no large-scale, high-quality human studies published as of 2024.

In summary, Astragaloside IV has scientific rationale and supportive preclinical research for peripheral neuropathy, but clinical validation in humans is insufficient. Thus, the evidence rating is moderate-low (2/5), and its use should be considered experimental pending further studies.
