Arginine is a semi-essential amino acid that plays a key role in nitric oxide (NO) production, which helps in vasodilation and increases blood flow. Its use in peripheral neuropathy is based on the hypothesis that improved microcirculation can benefit nerve health, particularly in conditions with compromised blood flow, such as diabetic neuropathy. A few small clinical studies and animal experiments have investigated arginine supplementation for neuropathy. For example, a 2014 study published in Diabetes Care found that oral L-arginine supplementation improved symptoms and nerve conduction in diabetic patients with peripheral neuropathy, likely due to enhanced endothelial function and NO-mediated vasodilation. However, the number of rigorous, large-scale human trials is limited, and results have been mixed. Some studies fail to show significant benefit over placebo.

Overall, while there is a plausible scientific rationale and some early evidence supporting arginine’s use for peripheral neuropathy (mainly in diabetes), the quality and quantity of data are insufficient for strong recommendations. Clinical guidelines do not currently endorse arginine for neuropathy treatment, and more research is needed to establish its efficacy and safety profile. Thus, the scientific evidence is emerging but not robust.
