Beta hydroxybutyrate (BHB) is a ketone body produced during periods of fasting, prolonged exercise, or ketogenic diets. Its use in Parkinson’s Disease (PD) is supported by preliminary scientific research, primarily in animal models and early human studies, but robust clinical evidence is limited. The rationale stems from the observation that mitochondrial dysfunction and impaired energy metabolism are hallmarks of PD. BHB can serve as an alternative energy substrate for neurons, potentially bypassing deficits in glucose metabolism seen in PD. Some animal studies have demonstrated that exogenous BHB administration improves motor function and reduces neurodegeneration in models of PD. Additionally, BHB has been shown to exert neuroprotective effects, such as reducing oxidative stress and inflammation, both implicated in PD pathophysiology. Small pilot human trials and case reports suggest possible symptomatic improvement, but these findings are preliminary and lack large-scale, randomized controlled trials to confirm efficacy or safety. Major reviews and consensus guidelines do not currently recommend BHB as a standard therapy for PD. Therefore, while there is a scientific basis and some early evidence for its potential benefit, the clinical evidence is currently insufficient for routine use. Further research, particularly well-designed human trials, is needed to evaluate BHB’s role in the management of Parkinson’s Disease.
