Omega-3 fatty acids, particularly EPA (eicosapentaenoic acid) and DHA (docosahexaenoic acid), have been investigated for their potential role in supporting bone health and treating osteoporosis. The rationale is based on their anti-inflammatory properties, as chronic inflammation is known to contribute to bone loss. Additionally, omega-3s may influence the balance between bone resorption and bone formation by modulating the activity of osteoclasts and osteoblasts.
Several animal studies have demonstrated that omega-3 supplementation can improve bone mineral density and bone strength. In humans, some observational studies suggest that higher dietary intake of omega-3s is associated with greater bone mineral density and reduced fracture risk. However, randomized controlled trials (RCTs) in humans have yielded mixed results. While a few small studies have shown modest benefits of omega-3 supplementation on bone turnover markers or density, most RCTs have not demonstrated significant improvements in clinically relevant bone outcomes.
Current evidence suggests a potential supportive role for omega-3 fatty acids in bone health, but the clinical relevance for osteoporosis prevention or treatment remains uncertain. Major guidelines do not recommend omega-3s as a primary therapy for osteoporosis. More large-scale, high-quality RCTs are needed to clarify their efficacy. Thus, while there is some scientific rationale and preliminary evidence, support for omega-3 fatty acids in treating osteoporosis is limited and not considered standard of care.
