Picamilon (N-nicotinoyl-GABA) is a synthetic compound developed in Russia that combines niacin (vitamin B3) and gamma-aminobutyric acid (GABA). It has been used in Russia as a prescription drug for neurological conditions, including neurosis, anxiety, and cerebrovascular disorders. The rationale for its use in neurosis is based on its purported ability to cross the blood-brain barrier and deliver GABA directly to the brain, potentially exerting anxiolytic and calming effects. Several Russian studies from the 1980s and 1990s have reported that Picamilon may reduce symptoms of anxiety, improve mood, and have mild stimulant effects without causing sedation. However, these studies are generally small, lack rigorous controls, and have not been widely replicated or reviewed in Western scientific literature.
Outside of Russia, there is limited peer-reviewed evidence supporting Picamilon's efficacy for neurosis or anxiety-related disorders. Most clinical data remain unpublished or are only available in Russian. Regulatory agencies in the United States and Europe do not recognize Picamilon as a medication, and it is not approved for medical use in these regions.
