Vitamin B6 (Pyridoxine HCl) has a documented scientific basis for use in certain types of neuralgia and neuritis, particularly those related to vitamin B6 deficiency. Pyridoxine is essential for normal nerve function, playing a role in neurotransmitter synthesis and myelin formation. Clinical evidence supports supplementation in cases where deficiency is present, such as in malnutrition, alcoholism, or isoniazid (a tuberculosis medication) therapy, all of which can cause peripheral neuropathy (a type of neuritis and neuralgia). In these cases, B6 supplementation can reverse symptoms.

However, for most cases of neuralgia and neuritis not linked to B6 deficiency, evidence supporting the use of B6 is limited and inconsistent. Some small studies and case reports suggest possible benefit in carpal tunnel syndrome and other neuropathies, but larger, high-quality randomized controlled trials are lacking. Moreover, excessive B6 intake can itself cause neuropathy, highlighting the need for caution.

In summary, while there is clear scientific rationale for using pyridoxine to treat neuralgia and neuritis when caused by deficiency, the evidence for its routine use in idiopathic or other forms of neuropathic pain is weak. Clinical guidelines generally recommend B6 supplementation only in cases of proven or suspected deficiency.
