Vitamin B1, also known as thiamine (and specifically in its active form as thiamine diphosphate or thiamine pyrophosphate), is scientifically recognized for its critical role in nerve function and carbohydrate metabolism. Its use in the support and treatment of neuralgia (nerve pain) and neuritis (nerve inflammation) is primarily based on its established role in preventing and treating deficiency states, such as beriberi and Wernicke-Korsakoff syndrome—both of which can present with neuropathic symptoms. Thiamine deficiency is known to cause peripheral neuropathy, which can manifest as pain, tingling, or weakness in the limbs. However, the evidence supporting the use of thiamine supplementation in neuralgia and neuritis outside of deficiency states is limited. Clinical studies are sparse, and most support comes from case reports or studies involving patients with documented thiamine deficiency. There are some small studies and anecdotal reports suggesting benefits in diabetic neuropathy and other neuropathic conditions, but high-quality randomized controlled trials are lacking. In summary, thiamine (including thiamine diphosphate) is scientifically justified in treating neuropathies that result from deficiency, but its effectiveness in neuralgia or neuritis of other etiologies is less well established. Routine use for neuralgia or neuritis in individuals without clear deficiency is not strongly supported by the scientific literature.
