Vitamin B1, also known as thiamine, and its derivative fursultiamine, have been used to support and treat neuralgia and neuritis, especially in the context of neuropathies associated with thiamine deficiency. There is scientific rationale for the use of thiamine in these conditions because thiamine plays a crucial role in nerve cell metabolism and function. Deficiency of thiamine can result in peripheral neuropathy, as seen in beriberi, which is characterized by symptoms of neuritis and neuralgia. Fursultiamine is a lipophilic thiamine derivative that enhances bioavailability and tissue penetration compared to standard thiamine salts, allowing for improved efficacy in replenishing thiamine levels in neural tissues.
While there is scientific support for thiamine supplementation in cases of deficiency-induced neuropathy, the evidence for its effectiveness in idiopathic neuralgia or neuritis, or those not related to deficiency, is limited. Some clinical studies and reviews have indicated symptomatic improvement in neuropathic pain and nerve inflammation with high-dose thiamine or its derivatives, but most of these studies are small, open-label, or not placebo-controlled. Fursultiamine is more widely used in Japan and some other Asian countries for neuropathic conditions, with some clinical data supporting its use for diabetic neuropathy and neuralgia, but robust, high-quality randomized controlled trials are lacking. Therefore, while there is a physiological and clinical rationale in deficiency states, and some scientific evidence for symptomatic benefit, the overall evidence base is moderate at best.
