Cocarboxylase, also known as thiamine pyrophosphate (TPP), is the active form of vitamin B1 (thiamine). Its use in the treatment or support of neuralgia and neuritis is primarily rooted in traditional medical practice rather than robust scientific evidence. Historically, vitamin B1 deficiency has been linked to neurological disorders such as beriberi and Wernicke-Korsakoff syndrome, which can present with neuropathic symptoms. Consequently, thiamine and its active forms have been used to support nerve health and manage neuralgias and neuritis, especially in cases where deficiency is suspected or confirmed. However, beyond the clear benefit in frank thiamine deficiency, there is limited direct clinical trial data supporting the efficacy of cocarboxylase specifically for neuralgia or neuritis in patients with normal thiamine levels. Most modern guidelines for neuralgia and neuritis do not include cocarboxylase or thiamine supplementation unless deficiency is documented. Thus, while its use persists in some countries and formulations, particularly in Eastern Europe, this is largely based on tradition and extrapolation from its role in deficiency states rather than on high-quality scientific validation for broader neuropathic indications.
