Tetrahydropalmatine (THP) is an alkaloid derived from several plants in the Corydalis and Stephania genera, frequently used in traditional Chinese medicine (TCM). Historically, THP-containing herbs have been employed in TCM formulations for their analgesic (pain-relieving) and sedative properties, particularly in the context of general pain and insomnia. While nerve pain (neuropathic pain) is not always distinctly separated from other pain types in traditional records, formulations containing Corydalis yanhusuo (which contains THP) have been used for conditions believed to involve nerve injury or dysfunction, such as numbness or neuralgia.
From a scientific perspective, there is limited but emerging evidence from animal studies suggesting that THP may have neuroprotective effects and could alleviate neuropathic pain via modulation of dopamine and other neurotransmitter systems. However, robust clinical trials in humans specifically targeting nerve damage or neuropathy are lacking. Most available studies focus on its general analgesic or sedative effects. Therefore, the routine use of THP for the treatment of nerve damage is rooted primarily in traditional use, with some preliminary scientific rationale but insufficient direct evidence for strong scientific validation.
