Nicotinamide riboside (NR) is a form of vitamin B3 and a precursor to nicotinamide adenine dinucleotide (NAD+), a molecule essential for cellular metabolism and energy production. Its use to support or treat nerve damage is primarily justified by emerging scientific evidence, mostly from preclinical studies. Animal models have demonstrated that NR supplementation can increase NAD+ levels in neurons, which is thought to be beneficial in conditions involving nerve damage, such as peripheral neuropathy or neurodegenerative diseases. NAD+ is crucial for neuronal survival, mitochondrial function, and axonal integrity; its depletion has been linked to neurodegeneration and impaired nerve repair.

Several studies in mice and rats have shown that NR can improve nerve regeneration, protect against chemotherapy-induced neuropathy, and reduce neuroinflammation. For example, NR has been shown to support the function of sirtuins and PARPs, enzymes involved in DNA repair and cellular stress responses. However, robust clinical trials in humans are still limited. A few early-stage human studies suggest that NR is safe and increases NAD+ in blood, but direct evidence of efficacy in treating nerve damage in people is still lacking. Therefore, while there is a promising scientific rationale and supporting preclinical data, clinical evidence remains insufficient, and the overall evidence strength is moderate at best, justifying a score of 2/5.
