Vitamin D is used to support patients with Myasthenia Gravis (MG) primarily based on scientific rationale, though the strength of evidence is moderate to low. MG is an autoimmune neuromuscular disorder characterized by weakness and fatigability of skeletal muscles. Vitamin D, beyond its well-known role in bone health, also has immunomodulatory properties that may be relevant in autoimmune diseases. Several observational studies have shown that vitamin D deficiency is relatively common in patients with MG. Some research suggests that lower vitamin D levels may be associated with increased disease severity or exacerbations in MG, likely due to vitamin D's influence on immune system regulation and T-cell function.

Despite these associations, high-quality randomized controlled trials specifically testing vitamin D supplementation as a treatment for MG are lacking. Most current recommendations for vitamin D in MG are extrapolated from general guidelines in autoimmune diseases and from the benefit of preventing osteoporosis, especially since long-term corticosteroid use (common in MG management) increases the risk of bone loss. Thus, vitamin D is often recommended as an adjunctive therapy for bone health and possibly to help modulate immune activity, but not as a primary treatment for MG itself. The evidence for direct symptomatic improvement in MG with vitamin D supplementation remains limited, justifying a moderate (2/5) evidence rating.
