Trans-geranylgeraniol is a naturally occurring diterpene alcohol involved in the biosynthesis of various biomolecules, including coenzyme Q10 (ubiquinone), which is essential for mitochondrial electron transport and ATP production. There is a growing body of preclinical evidence suggesting that geranylgeraniol and related compounds may play a role in supporting mitochondrial function. Specifically, studies in cell cultures and animal models have shown that geranylgeraniol supplementation can restore deficits in coenzyme Q10 biosynthesis, particularly in the context of statin-induced mitochondrial dysfunction, as statins inhibit the mevalonate pathway, reducing both cholesterol and coenzyme Q10 levels. Despite these promising findings, direct clinical evidence in humans demonstrating that trans-geranylgeraniol supplementation improves mitochondrial health or function is limited. Most of the data are indirect or based on mechanistic studies rather than robust clinical trials.
