Pyridoxal-5-phosphate (P-5-P) is the active coenzyme form of vitamin B6 and plays a critical role in cellular metabolism, including within the mitochondria. Scientific evidence supports its importance in mitochondrial function primarily because P-5-P is an essential cofactor for numerous enzymatic reactions involved in amino acid metabolism, neurotransmitter synthesis, and, importantly, energy production pathways such as the citric acid (Krebs) cycle. Several mitochondrial enzymes, including aminotransferases and those involved in the metabolism of homocysteine, require P-5-P for optimal activity. Deficiency in P-5-P has been associated with impaired mitochondrial function, increased oxidative stress, and decreased production of ATP, the cell's main energy currency.

Multiple studies in cell and animal models have shown that adequate levels of vitamin B6 and P-5-P are necessary for mitochondrial health and function. For example, research has linked low P-5-P status to mitochondrial dysfunction in neurodegenerative disorders and metabolic diseases. Clinical evidence in humans is more limited but consistent with these mechanistic insights, as supplementation with vitamin B6 or P-5-P can improve metabolic outcomes in certain deficiency states and rare mitochondrial disorders. Overall, while not universally used as a direct “mitochondrial supplement,” P-5-P’s role in mitochondrial health is well-supported at the biochemical level, and there is moderate-to-strong scientific justification for its use to support mitochondrial function in the context of deficiency or increased metabolic demand.
