Medium Chain Triglycerides (MCTs) are supported by scientific evidence for their role in supporting mitochondrial function, particularly through their unique metabolic pathway. MCTs are more rapidly absorbed and metabolized than long-chain triglycerides, as they do not require bile salts for digestion and are transported directly to the liver via the portal vein. In the liver, MCTs are quickly converted into ketone bodies, which serve as alternative energy substrates for the brain and other tissues, especially when glucose availability is limited. Ketone bodies are considered efficient fuels for mitochondrial energy production, and emerging research suggests they may enhance mitochondrial biogenesis, function, and protect against oxidative stress.

Several studies have investigated MCT supplementation in the context of neurological disorders such as Alzheimer's disease, where mitochondrial dysfunction is implicated. For example, MCT supplementation has been shown to increase blood ketone levels and improve cognitive function in some patients with mild to moderate Alzheimer's disease (Reger et al., 2004; Henderson et al., 2009). Animal studies and in vitro models also indicate that MCTs may promote mitochondrial energy production and reduce markers of mitochondrial dysfunction.

However, while the mechanistic rationale and early human and animal data are promising, large-scale clinical trials are limited, and much of the evidence comes from indirect or disease-specific models. Therefore, the overall evidence rating is moderate, with further research needed to clarify the direct impact of MCTs on mitochondrial health in the general population.
