Idebenone is a synthetic analog of coenzyme Q10 (ubiquinone) and is used to support mitochondrial function, primarily based on its role as a potent antioxidant and electron carrier in the mitochondrial respiratory chain. Scientific evidence supporting idebenone’s use for mitochondrial support is moderate and comes mainly from studies on rare mitochondrial disorders, such as Leber’s Hereditary Optic Neuropathy (LHON) and Friedreich’s ataxia. Clinical trials have shown that idebenone can improve visual function in some LHON patients and may have modest benefits in other mitochondrial diseases. The mechanism proposed involves idebenone bypassing dysfunctional complexes in the mitochondrial electron transport chain, thereby improving cellular energy production and reducing oxidative stress. However, evidence for its use in healthy individuals or for general “mitochondrial support” outside of these specific conditions is limited. Regulatory agencies in Europe have approved idebenone for certain mitochondrial disorders, but not for general health use. Overall, while preclinical data and some clinical studies provide a scientific rationale for idebenone’s use in supporting mitochondrial function, the breadth and strength of evidence are currently limited to specific patient populations, and its efficacy in broader applications remains unproven.
