Progesterone is scientifically validated for use in the prevention of miscarriage in specific circumstances. Progesterone is a hormone crucial for preparing and maintaining the uterine lining, supporting early pregnancy. Scientific evidence, particularly from randomized controlled trials and systematic reviews, supports its use for women with a history of recurrent miscarriage or those experiencing threatened miscarriage with evidence of luteal phase deficiency. The most notable evidence comes from the 2019 PRISM trial and subsequent meta-analyses, which found that vaginal progesterone supplementation modestly improved live birth rates in women with early pregnancy bleeding and a history of previous miscarriage. The benefit is most apparent in women who have had multiple (three or more) previous miscarriages. However, progesterone is not universally effective for all causes of miscarriage, as many miscarriages are due to chromosomal abnormalities, which progesterone cannot prevent.
Clinical guidelines, such as those from the American College of Obstetricians and Gynecologists (ACOG) and the UK’s NICE, recommend considering progesterone in specific high-risk populations, but not for all women.
