There is emerging scientific interest in the potential role of vitamin D in the prevention and treatment of migraine, though evidence is still limited and mixed. Several observational studies have found that individuals with migraine are more likely to have low serum vitamin D levels compared to healthy controls. Some small randomized controlled trials (RCTs) have suggested that vitamin D supplementation may reduce the frequency and severity of migraine attacks, particularly in patients who are deficient in vitamin D. Proposed mechanisms include vitamin D’s anti-inflammatory effects, modulation of neurotransmitters, and influence on calcium homeostasis, all of which could theoretically impact migraine pathophysiology.

However, the overall quality of evidence is moderate to low. Meta-analyses of available trials indicate a possible modest benefit, but results are inconsistent and often limited by small sample sizes, short follow-up periods, and heterogeneity in study design. Current clinical guidelines do not universally recommend vitamin D supplementation specifically for migraine prevention, except in cases where vitamin D deficiency is documented. Thus, while there is some scientific rationale and preliminary evidence supporting the use of vitamin D in migraine management, robust, large-scale RCTs are needed to establish its efficacy and optimal dosing.
