Terpene lactones, particularly ginkgolides and bilobalide, are active compounds found in Ginkgo biloba extract. Their use for supporting memory and brain function is scientifically supported, though the evidence is moderate rather than definitive. Numerous clinical trials and meta-analyses have investigated Ginkgo biloba extract, which contains terpene lactones, for cognitive enhancement and the treatment of dementia and mild cognitive impairment. Studies suggest that these compounds may improve cerebral blood flow, inhibit platelet-activating factor, exhibit antioxidant properties, and protect neurons from oxidative stress and apoptosis, all of which are mechanisms relevant to brain health and memory. Some randomized controlled trials have demonstrated mild to moderate improvements in cognitive function in elderly individuals and those with dementia when compared to placebo. However, results are mixed, with some large, well-controlled studies finding little to no effect, especially in healthy adults. Major medical organizations, such as the American Academy of Neurology, generally regard the evidence as insufficient for routine clinical use but acknowledge some potential benefit in specific populations. Overall, terpene lactones are widely used in Europe and Asia as adjuncts in cognitive decline, with safety profiles considered acceptable. More robust, large-scale trials are necessary to provide conclusive evidence.
