Pentacyclic Oxindole Alkaloids (POAs) are bioactive compounds primarily found in the plant Uncaria tomentosa, commonly known as Cat's Claw. Their use for supporting or treating lupus (systemic lupus erythematosus, SLE) is rooted mostly in traditional herbal medicine, particularly within South American indigenous medical practices. Cat's Claw has a long history of use as a general immune modulator and anti-inflammatory agent in folk medicine; however, there is scant direct historical evidence specifically linking its use to lupus management.

From a scientific perspective, there are some laboratory studies showing that POAs possess immunomodulatory and anti-inflammatory properties, such as inhibiting TNF-α and regulating T-cell activity. However, these studies are mostly in vitro or in animal models and do not specifically address lupus. There is a lack of high-quality clinical trials or robust scientific studies that evaluate the efficacy or safety of POAs in human lupus patients. The only supportive evidence is extrapolated from their general immune-modulating effects, not from lupus-specific research.

Therefore, the use of POAs for lupus is primarily justified by traditional rather than scientific evidence, and the quality of evidence specific to lupus is very low. Patients should exercise caution and consult healthcare professionals before considering such supplements, as autoimmune conditions like lupus require carefully managed treatment plans.
