Kaolin, a naturally occurring clay mineral, has a long history of traditional use in supporting the health of the large intestine (colon), primarily in the management of diarrhea. Its use dates back to ancient medicinal systems, including traditional Chinese medicine and 19th–20th century Western pharmacopeias. Kaolin acts as an adsorbent, binding to toxins and excess water in the gastrointestinal tract, which can help reduce stool liquidity and frequency. In the 20th century, kaolin was a key component in over-the-counter antidiarrheal products, often combined with pectin, such as in the well-known formulation "Kaopectate." However, scientific evidence supporting its efficacy is limited, with most modern clinical guidelines now favoring other antidiarrheal agents due to concerns about effectiveness and potential for masking serious conditions. Recent systematic reviews and regulatory bodies have generally concluded that while kaolin may have some mild adsorbent properties, its benefit over placebo in treating diarrhea is not robustly demonstrated in controlled clinical trials. Still, its traditional use remains well-documented, particularly as a gentle, non-systemic remedy for mild gastrointestinal disturbances. Kaolin is no longer widely recommended in evidence-based medicine for large intestine support, but its historical usage persists in some cultures and alternative medicine practices.
