Bifidobacterium species are among the most studied probiotics with respect to lactose intolerance. Evidence suggests that certain strains of Bifidobacterium can improve lactose digestion and tolerance in individuals who are lactose intolerant. The primary mechanism is the production of β-galactosidase (lactase) by Bifidobacterium, which helps break down lactose in the gut, partially compensating for host lactase deficiency. Several randomized controlled trials and in vitro studies have shown that supplementation with Bifidobacterium-containing probiotics can reduce symptoms such as bloating, flatulence, and diarrhea after lactose ingestion.

However, the majority of these studies use mixed probiotic formulations (e.g., with Lactobacillus species), and relatively few focus on Bifidobacterium alone. The degree of benefit appears to be strain-dependent and not all Bifidobacterium strains have demonstrated efficacy. Meta-analyses conclude that while there is moderate evidence to support the use of probiotics—including Bifidobacterium—for lactose intolerance, the effects are generally modest and more robust, strain-specific research is needed.
