Spiny Restharrow (Ononis spinosa) has a long history of use in traditional European herbal medicine, particularly for urinary tract ailments, including kidney infections and inflammation. Historically, its roots and aerial parts were used as diuretics and to support kidney and bladder health. Herbalists believed that the plant promoted urine flow and helped "flush out" infections, which underlies its traditional application for kidney infections (pyelonephritis) and other urinary tract issues. The German Commission E has approved Spiny Restharrow for use in irrigation therapy for inflammatory diseases of the urinary tract and for preventing kidney gravel (small kidney stones), reflecting its acceptance in traditional and phytotherapeutic contexts.

However, scientific evidence supporting Spiny Restharrow’s effectiveness specifically for kidney infections is limited. Some in vitro studies show mild antibacterial or anti-inflammatory effects, but these are preliminary and do not directly validate its clinical use in kidney infections. The diuretic effect is more substantiated, which may provide symptomatic relief or help prevent urinary stasis, but does not equate to treating an active infection. There are no robust clinical trials demonstrating efficacy against kidney infections, nor is there evidence that it can replace antibiotics or medical management. Therefore, its use is primarily justified by tradition and not modern scientific validation.
