Berberis species, including Berberis vulgaris (barberry), have a long history of traditional use in herbal medicine for various infections, including those affecting the urinary tract and kidneys. Traditional systems of medicine such as Ayurveda and traditional Persian medicine have used Berberis extracts or berberine-containing preparations to support kidney and urinary tract health, often citing their “cooling” and infection-fighting properties. The primary active constituent, berberine, has demonstrated antimicrobial activity in laboratory studies, particularly against bacteria commonly implicated in urinary tract infections. However, most clinical evidence for Berberis or berberine relates to general antimicrobial or anti-inflammatory effects, with only limited direct study on kidney infections (pyelonephritis) specifically. There are no robust, well-designed clinical trials validating Berberis for kidney infection treatment in humans, and its use remains largely rooted in traditional practice and extrapolation from broader antimicrobial effects. Therefore, while some laboratory and animal studies suggest a plausible basis for its use, the evidence is not strong or direct. Use of Berberis for kidney infections should not replace conventional medical treatment, as untreated kidney infections can be serious or life-threatening.
