S-adenosylmethionine (SAM-e) has been investigated for its potential benefits in supporting joint health, particularly in the management of osteoarthritis. Several clinical studies and systematic reviews have examined its efficacy. SAM-e appears to have anti-inflammatory and analgesic properties, which may alleviate joint pain and improve function. A number of randomized controlled trials (RCTs) have compared SAM-e to nonsteroidal anti-inflammatory drugs (NSAIDs) and placebo. Some studies suggest that SAM-e provides similar pain relief to NSAIDs, with fewer gastrointestinal side effects. For example, a 2002 meta-analysis published in BMC Musculoskeletal Disorders concluded that SAM-e was more effective than placebo and as effective as NSAIDs for reducing pain and improving functional limitation in osteoarthritis patients. The proposed mechanism involves SAM-e’s role in cartilage proteoglycan synthesis and anti-inflammatory methylation reactions.
However, while evidence favors its use over placebo and indicates comparability to NSAIDs, the quality of studies is variable, with some having small sample sizes and methodological limitations. As such, while SAM-e is scientifically validated to some extent for joint support, the evidence is moderate rather than strong. Clinical guidelines do not routinely recommend its use, but it is considered an option for patients who cannot tolerate NSAIDs. Thus, SAM-e’s use for joint support is supported by moderate scientific evidence but requires further large-scale, high-quality trials for definitive conclusions.
