Shiitake mushrooms (Lentinula edodes) have been traditionally consumed in East Asia for their culinary and medicinal properties, but their use in Inflammatory Bowel Disorders (IBD) such as Crohn’s disease and ulcerative colitis is primarily supported by emerging scientific evidence rather than long-standing tradition. Preclinical studies have shown that shiitake contains bioactive compounds, notably polysaccharides like beta-glucans and lentinan, which exhibit immunomodulatory and anti-inflammatory effects. In animal models of colitis, shiitake mushroom extracts have been demonstrated to reduce inflammation, improve gut barrier function, and modulate the composition of the gut microbiota. These effects are thought to be mediated by downregulation of pro-inflammatory cytokines (e.g., TNF-α, IL-6) and enhancement of anti-inflammatory pathways.

However, clinical data in humans remain limited. Few small-scale pilot studies or case reports have explored shiitake supplementation in people with IBD, with some suggesting potential benefits but lacking robust, placebo-controlled randomized trials. As such, while there is a scientific rationale for shiitake’s anti-inflammatory properties and some supportive preclinical findings, the clinical evidence base is not strong enough to recommend its use as a primary or adjunctive therapy for IBD. More high-quality human research is needed to ascertain efficacy, optimal dosages, and safety profiles. Thus, the current evidence rating is moderate to low (2/5), reflecting promising laboratory data but insufficient clinical validation.
