N-acetylcysteine (NAC) is an antioxidant and precursor to glutathione, widely studied for its effects on oxidative stress and inflammation. Its use in Inflammatory Bowel Disorders (IBD), such as Crohn’s disease and ulcerative colitis, is supported by some scientific evidence, primarily from preclinical studies and a small number of clinical trials. NAC is thought to help mitigate the oxidative damage and inflammatory processes that are central to IBD pathophysiology by replenishing intracellular glutathione and scavenging reactive oxygen species.
Animal studies have shown that NAC can reduce colonic inflammation, tissue damage, and markers of oxidative stress in models of IBD. Some small clinical studies and pilot trials in humans suggest that NAC supplementation may improve certain clinical symptoms and reduce inflammation, especially when used as an adjunct to conventional therapy. However, these studies are limited by small sample sizes, short durations, and heterogeneity in dosing regimens.
Currently, there is not enough robust clinical evidence to recommend NAC as a stand-alone or primary therapy for IBD patients. The evidence supporting its use is preliminary, and large-scale, high-quality randomized controlled trials are lacking. Therefore, while there is a scientific rationale and early research suggesting potential benefit, the overall evidence base remains limited, justifying a relatively low evidence rating.
