L-glutamine is an amino acid that serves as a primary fuel source for enterocytes (cells lining the intestines) and plays a role in maintaining gut barrier function. Its use in supporting or treating Inflammatory Bowel Disorders (IBD), such as Crohn’s disease and ulcerative colitis, is based on its physiological importance in intestinal health and mucosal repair. Several animal studies and limited human trials have indicated that glutamine supplementation may help reduce intestinal inflammation, enhance mucosal healing, and improve intestinal permeability in IBD models. However, the current clinical evidence in humans is mixed and generally of low to moderate quality.
A few small-scale clinical trials have shown modest benefits of glutamine supplementation in patients with IBD, primarily in improving some markers of gut barrier function and reducing inflammation. However, other studies have failed to show significant improvement in clinical outcomes such as symptom reduction or remission rates. Meta-analyses and systematic reviews generally conclude that while glutamine is safe and may have a biological rationale for use, robust evidence for its efficacy in IBD patients is lacking. Major gastroenterology guidelines do not currently recommend glutamine supplementation as a standard therapy for IBD.
In summary, while there is a scientific rationale and some preliminary evidence for glutamine’s potential benefits in IBD, current clinical data does not strongly support its routine use, resulting in a relatively low evidence rating.
