Glycomacropeptide (GMP) is a bioactive peptide derived from κ-casein during cheese production and has attracted interest for its potential health benefits, including in the management of Inflammatory Bowel Disorders (IBD) such as Crohn’s disease and ulcerative colitis. The scientific rationale for GMP’s use in IBD is based on its demonstrated anti-inflammatory and prebiotic properties in preclinical studies. GMP has been shown to modulate gut microbiota composition by promoting beneficial bacteria (e.g., bifidobacteria and lactobacilli) and inhibiting pathogens. In animal models of colitis, GMP supplementation has led to reductions in inflammatory cytokines (e.g., TNF-α, IL-1β) and improvements in mucosal integrity, suggesting a protective effect on the gut barrier. Mechanistically, GMP may inhibit immune cell activation and reduce oxidative stress in the gut. However, the evidence supporting GMP’s use in IBD remains limited to in vitro and animal studies, with very few small-scale human trials. Most human research has focused on its nutritional and metabolic effects, not specifically on IBD. While preliminary findings are promising, there is insufficient high-quality clinical evidence to fully endorse GMP as a treatment for IBD. Thus, while scientific interest exists and early data support potential benefits, more robust, large-scale human clinical trials are needed to establish efficacy and safety in IBD patients.
