Boswellic acids are bioactive compounds derived from the resin of Boswellia serrata (Indian frankincense). Their use in the management of inflammatory bowel disorders (IBD), such as Crohn's disease and ulcerative colitis, is supported by both traditional medicine and emerging scientific research. Preclinical studies demonstrate that boswellic acids possess anti-inflammatory properties, primarily by inhibiting 5-lipoxygenase, which reduces leukotriene synthesis, a pathway implicated in gut inflammation. Several small randomized controlled trials (RCTs) in humans have evaluated standardized Boswellia extracts in patients with ulcerative colitis. For example, a 1997 double-blind study (Gupta et al.) reported that 82% of patients achieved remission with Boswellia gum resin compared to 75% with sulfasalazine. Other studies have shown improvements in symptoms and histological markers of inflammation, with a generally favorable safety profile. However, these studies are limited by small sample sizes, short duration, and variable product quality and standardization. Major clinical guidelines do not yet recommend Boswellic acids as first-line therapy for IBD, but they may be considered as adjunctive therapy, especially in patients seeking alternative approaches or those intolerant to conventional medications. Thus, while there is preliminary scientific validation for the use of Boswellic acids in inflammatory bowel disorders, larger and more rigorous studies are needed to fully establish their efficacy and safety profile.
