Umckalin is a coumarin compound found in the roots of Pelargonium sidoides, a plant traditionally used in South African medicine for treating respiratory infections. Modern scientific interest in Umckalin is primarily due to its presence in herbal extracts such as Umckaloabo (EPs® 7630), which have been studied for their effects on bacterial infections, particularly those affecting the respiratory tract.

Several clinical trials and in vitro studies have demonstrated that extracts containing Umckalin possess antibacterial, antiviral, and immunomodulatory properties. These extracts appear to inhibit the adhesion of bacteria to mucous membranes, modulate immune responses by enhancing phagocytosis, and may promote the clearance of infected mucus. Systematic reviews and meta-analyses of EPs® 7630 have shown improved recovery times and symptom relief in patients with acute bronchitis and other upper respiratory tract infections compared to placebo, although many studies are of moderate quality and often use the whole extract rather than isolated Umckalin.

While Umckalin is not the sole active constituent, and direct evidence for Umckalin alone is limited, the overall scientific support for Pelargonium sidoides root extract (which includes Umckalin) in treating bacterial respiratory infections is moderate. Thus, its use for this indication is supported by scientific evidence, though more high-quality research on Umckalin specifically is needed.
