Lithospermum erythrorhizon, commonly known as purple gromwell or "Zi Cao" in traditional Chinese medicine, has been used historically for its purported anti-inflammatory and antimicrobial properties. Scientific studies in recent decades have begun to validate some of these traditional uses, particularly regarding its antibacterial activity. The root of L. erythrorhizon contains shikonin and its derivatives, which have been shown in vitro to possess significant antibacterial effects against a range of Gram-positive and some Gram-negative bacteria, including Staphylococcus aureus and Escherichia coli. Several laboratory studies have demonstrated that shikonin disrupts bacterial cell membranes and inhibits biofilm formation, lending support to its traditional use in the treatment of skin infections and wounds.

However, most of the evidence is preclinical, consisting of in vitro assays and animal models, with few well-designed human clinical trials available. While the traditional use of L. erythrorhizon for infected wounds and ulcers has been documented in East Asian medical literature, the scientific validation for its systemic antibacterial effects in humans remains limited. Nonetheless, the consistency of antimicrobial activity in laboratory studies warrants a moderate evidence rating. Further research, especially clinical trials, is needed to fully establish its efficacy and safety for treating bacterial infections in humans.
