Brevibacillus brevis is a Gram-positive, spore-forming bacterium known primarily for its ability to produce a range of antimicrobial compounds, most notably the antibiotic tyrothricin, which is a mixture of gramicidin and tyrocidine. The discovery and subsequent study of these compounds, particularly during the early and mid-20th century, provided scientific validation for the use of B. brevis and its products as antibacterial agents. Tyrothricin and its components are effective against a variety of Gram-positive bacteria and have seen use in topical treatments for minor skin infections and throat lozenges. However, their application is limited to topical or local use due to potential toxicity when administered systemically.

The evidence supporting B. brevis for treating bacterial infections comes largely from in vitro studies and clinical use of its antimicrobial peptides, rather than direct use of the live bacteria itself. Various studies have characterized the mechanisms of these peptides, confirming their bactericidal activity and potential in novel antibiotic development. Nonetheless, the direct use of B. brevis as a probiotic or therapeutic agent for bacterial infection in humans is not common practice, and clinical evidence remains limited to its secondary metabolites. Thus, while there is scientific validation for its antibacterial compounds, the evidence is moderate (rated 3/5) due to the lack of direct clinical trials using the organism itself as a therapeutic for infections.
