Polygonum cuspidatum root, also known as Japanese knotweed, is traditionally used in East Asian herbal medicine for various ailments, including infections. Scientifically, its primary active constituent, resveratrol, as well as other polyphenols, have demonstrated antimicrobial and antiviral properties in laboratory studies. In vitro experiments have shown that extracts of cuspidatum root can inhibit the growth of certain bacteria, such as Staphylococcus aureus and Escherichia coli, and exhibit activity against viruses like herpes simplex virus. The plant also contains emodin, which has shown some inhibitory effects on viral replication in cell cultures. However, while these laboratory findings suggest potential antimicrobial effects, there is a lack of high-quality clinical trials in humans confirming its efficacy for treating infections. Most evidence is limited to cell culture and animal studies, and the bioavailability and safety of these compounds in humans at therapeutic doses remain unclear. As such, while there is a scientific rationale and preliminary research indicating possible effectiveness, the evidence is not robust or well-established for clinical use in infection. Therefore, the evidence rating is 2 out of 5, denoting weak but plausible scientific support.
