Alantolactone is a sesquiterpene lactone compound primarily found in the roots of Inula helenium (elecampane). Its traditional use in herbal medicine has centered around respiratory ailments rather than infection specifically. However, recent years have seen an increase in scientific studies examining its antimicrobial properties.

In vitro studies have demonstrated that alantolactone exhibits antibacterial and antifungal activities. For example, research shows inhibitory effects against Staphylococcus aureus (including MRSA), Escherichia coli, and certain pathogenic fungi such as Candida albicans. The proposed mechanisms include disruption of microbial cell membranes, inhibition of biofilm formation, and interference with microbial enzyme systems.

Despite these promising laboratory results, there is a lack of high-quality clinical evidence in humans supporting the use of alantolactone for the treatment of infections. Most studies are preclinical, and there are no robust clinical trials evaluating efficacy, safety, or dosing in humans. Thus, while there is a scientific basis for potential antimicrobial effects, current evidence does not justify recommending alantolactone as a standalone treatment for infections in clinical practice. Further research, particularly human trials, is needed to establish its efficacy and safety profile.
