Colocynth (Citrullus colocynthis), also known as bitter apple, has a long history of use in traditional medicine systems, particularly in the Middle East, North Africa, and parts of Asia. Historically, the pulp or extract of colocynth has been used as a cathartic (strong laxative) and was thought to aid in the relief of digestive complaints, including indigestion, by promoting bowel movements. Ancient texts, such as those from Greek, Roman, and Islamic physicians, mention colocynth as a remedy for various gastrointestinal issues. However, its effects are harsh, and traditional sources also warn of its potential for causing severe irritation, cramping, and even poisoning when used in excess.

Modern scientific research on colocynth for indigestion is limited and generally not supportive, as most studies focus on its toxicity rather than its efficacy for digestive complaints. The fruit is known to contain potent compounds such as cucurbitacins, which are responsible for both its pharmacological and toxic effects. Due to the risk of severe side effects, including gastrointestinal inflammation and damage, colocynth is not recommended in evidence-based medicine for indigestion. Its use persists mainly in folk and traditional practices, with little to no scientific validation to support its safety or effectiveness for this purpose.
