Vitamin B6 (pyridoxine) has been traditionally used in some cases to manage persistent or intractable hiccups, although robust scientific evidence is limited. The rationale comes from several case reports and small case series, particularly in the context of patients who develop hiccups as a side effect of medications (e.g., corticosteroids, chemotherapy), or in relation to neurological disorders. The traditional use is partly based on the role of vitamin B6 in neurotransmitter synthesis, especially gamma-aminobutyric acid (GABA), which is involved in inhibitory pathways in the central nervous system. A few case reports, such as those documented in the 1950s and 1970s, describe relief from hiccups after administration of vitamin B6, especially in the context of drug-induced hiccups. However, there are no large-scale randomized controlled trials or systematic reviews that confirm efficacy, and the existing evidence is largely anecdotal. Modern clinical guidelines do not routinely recommend vitamin B6 for hiccups, except perhaps for specific cases where deficiency or drug interaction is suspected. Therefore, while there is a tradition and some low-level evidence supporting its use, scientific validation is weak, and vitamin B6 should not be considered a first-line therapy for hiccups.
