Lithospermum erythrorhizon, commonly known as "Zi Cao" in traditional Chinese medicine, has been used historically for a variety of skin conditions. In recent decades, some scientific studies have examined its potential antiviral properties, particularly against herpes simplex virus (HSV). The primary active compound, shikonin, and related naphthoquinone derivatives have demonstrated inhibitory effects on HSV in vitro. For example, research published in Antiviral Research and other journals indicates that extracts from L. erythrorhizon can suppress HSV-1 replication in cultured cells, likely by interfering with viral gene expression and replication mechanisms. However, these findings are mostly limited to laboratory (cell culture) studies, with very limited animal data and almost no high-quality clinical trials in humans. While these results suggest a possible scientific basis for the use of L. erythrorhizon in managing herpes infections, the current evidence is preliminary. No robust clinical trials have confirmed its efficacy or safety for this indication. Thus, while its traditional use, especially in East Asian medicine, aligns with treating skin lesions that might include herpes, the scientific support is limited and not yet sufficient for strong clinical recommendations.
