Aloe vera has been investigated for its potential benefits in the management of herpes simplex virus (HSV) infections, particularly in the context of topical application. The interest in Aloe vera stems from its known anti-inflammatory, wound-healing, and potential antiviral properties. Small clinical studies have evaluated Aloe vera cream for the treatment of genital herpes. For example, a double-blind, placebo-controlled study published in 1999 (Syed et al., J Dermatolog Treat) found that 0.5% Aloe vera cream significantly increased the healing rate of genital herpes lesions compared to placebo. Other studies are limited in size and scope, but suggest a possible benefit in reducing lesion healing time and symptom severity. Mechanistically, Aloe vera contains compounds such as acemannan and anthraquinones, which have shown some antiviral activity in vitro. However, the evidence base is weak: there are few high-quality, large-scale randomized controlled trials, and most studies suffer from small sample sizes and limited follow-up. While Aloe vera is traditionally used for skin conditions and wound healing, its use specifically for herpes is not a long-standing traditional remedy but rather a subject of recent scientific inquiry. Overall, Aloe vera may offer some benefit as a topical adjunct for herpes lesions, but its efficacy is not robustly established, and it should not replace standard antiviral therapies.
