Vitamin E has been investigated as a supportive therapy for hepatitis, particularly non-alcoholic steatohepatitis (NASH), a form of chronic liver inflammation. The rationale for its use is based on its antioxidant properties, as oxidative stress is thought to play a significant role in hepatocellular injury in various forms of hepatitis. Several randomized controlled trials, most notably the PIVENS trial (N Engl J Med 2010;362:1675-85), have evaluated the effectiveness of vitamin E supplementation in adults with NASH but without diabetes. These studies observed modest improvements in liver histology, particularly in reducing hepatic steatosis and inflammation, though not necessarily fibrosis. While some improvement in biochemical markers and liver histology has been reported, vitamin E is not universally effective and does not replace standard antiviral or disease-specific therapies for viral hepatitis (hepatitis B or C). The American Association for the Study of Liver Diseases (AASLD) recommends considering vitamin E in non-diabetic adults with biopsy-proven NASH, but not for other forms of hepatitis. Concerns also exist about the long-term safety of high-dose vitamin E supplementation, including potential increased risks of all-cause mortality and hemorrhagic stroke at high doses. In summary, scientific evidence for vitamin E's use is modest and condition-specific, supporting its use primarily in non-diabetic adults with NASH, but not as a treatment for viral hepatitis or other forms of hepatitis.
