Cholic acid is a primary bile acid naturally produced by the liver and plays a key physiological role in the digestion and absorption of dietary fats and fat-soluble vitamins. Its use as a therapeutic agent is scientifically validated, particularly in the management of certain rare inherited disorders of bile acid synthesis, such as bile acid synthesis disorders due to single enzyme defects (e.g., 3β-hydroxy-Δ⁵-C27-steroid oxidoreductase deficiency and Δ⁴-3-oxosteroid 5β-reductase deficiency). Clinical studies and regulatory approvals (such as the FDA approval of cholic acid under the brand name Cholbam) confirm that supplementation with cholic acid can restore normal bile acid levels, improve liver function, and prevent progression to liver failure in affected patients. The evidence base consists of clinical trials demonstrating improvements in liver biochemical markers, growth, and survival in pediatric and adult patients with these rare disorders. However, the use of cholic acid as a general hepatic support supplement for the broader population is not well established, and its benefits outside of these specific medical indications remain unproven. Thus, while there is strong scientific validation for its use in supporting the hepatic system in certain rare conditions, routine use for general liver health lacks robust evidence.
