Sarsaparilla (from species such as Smilax officinalis and related plants) has a long history of use in traditional herbal medicine systems, including those of the Americas, Europe, and Asia. Its roots have been used for a variety of conditions, most notably as a "blood purifier," for skin diseases, and for rheumatism. In some traditional systems, sarsaparilla was included in herbal mixtures intended to address hemorrhoids, based on its reputed anti-inflammatory and detoxifying properties. However, this connection is primarily due to anecdotal and traditional beliefs rather than a direct, consistent historical link specifically to hemorrhoid treatment.

There is very little scientific evidence supporting the use of sarsaparilla for hemorrhoids. Modern research on sarsaparilla has focused mainly on its possible anti-inflammatory, antibacterial, and antioxidant activities, but these effects have not been specifically studied in the context of hemorrhoidal disease. No robust clinical trials or preclinical studies have validated its effectiveness for this indication. Thus, while the herb appears in some traditional mixtures for hemorrhoids, the evidence for its efficacy is extremely weak and based almost entirely on historical use rather than scientific validation.
