Flavanols are a group of plant-based polyphenols found in foods like cocoa, tea, apples, and berries. Their use in managing hemorrhoids is primarily supported by scientific evidence, particularly when administered as part of flavonoid mixtures (such as micronized purified flavonoid fraction, MPFF) rather than isolated flavanols.

Several randomized controlled trials and meta-analyses have assessed the efficacy of flavonoid mixtures in alleviating symptoms of hemorrhoids, including bleeding, pain, itching, and recurrence. These mixtures, often containing diosmin, hesperidin, and other related flavanols, are thought to improve venous tone, reduce capillary permeability, and exert anti-inflammatory effects, which can be beneficial in hemorrhoidal disease.

A 2012 Cochrane review and later systematic reviews concluded that flavonoid mixtures can reduce the risk of persistent symptoms and recurrence compared to placebo, with a good safety profile. However, much of the research is on mixed flavonoid preparations rather than isolated flavanols, and there is some heterogeneity in study quality and outcome measures.

In summary, there is moderate-quality scientific evidence supporting the use of flavanol-containing mixtures in the management of hemorrhoids, though not specifically for isolated flavanols. Clinical guidelines in some countries recommend their use as adjunct therapy for acute hemorrhoidal attacks.
