Vitamin B9, also known as folic acid (synthetic form) or folate (natural form), has some scientific validation for its role in supporting heart health, primarily through its impact on homocysteine metabolism. Elevated homocysteine levels are considered a risk factor for cardiovascular diseases, including heart weakness and failure. Folic acid, along with vitamins B6 and B12, helps convert homocysteine into methionine, thereby lowering homocysteine concentrations in the blood. Several studies and meta-analyses have shown that folic acid supplementation can reduce homocysteine levels, which theoretically may translate into reduced cardiovascular risk.

However, the evidence linking folic acid supplementation directly to improved heart strength or reduced incidence of heart failure is mixed and generally modest. Large randomized controlled trials, such as the HOPE-2 trial, have demonstrated that while folic acid lowers homocysteine, it does not significantly reduce the risk of major cardiovascular events in populations without folate deficiency. In regions with mandatory folic acid food fortification, additional supplementation has not shown strong protective effects. That said, in populations with low baseline folate levels (such as in China before fortification), some studies have found a modest reduction in stroke and cardiovascular events with supplementation.

In summary, while there is a plausible biological rationale and some evidence supporting folic acid’s role in cardiovascular health, especially in folate-deficient populations, the benefit specifically for "heart weakness" (heart failure) is not robustly established. The evidence is stronger for reducing homocysteine rather than clear clinical outcomes.
