Vitamin E, specifically in the form of mixed tocopherols, has been investigated for its potential role in supporting heart health, particularly due to its antioxidant properties. The rationale is that oxidative stress contributes to atherosclerosis and other cardiovascular diseases; thus, antioxidants like vitamin E might confer protective effects. Early epidemiological studies suggested that higher vitamin E intake was associated with reduced risk of coronary heart disease. However, several large randomized controlled trials, such as the HOPE and Heart Protection Study, failed to show significant benefit of vitamin E supplementation in reducing cardiovascular events or mortality in people with heart disease or at risk for it. Some studies even suggested potential harm or increased risk of heart failure with high-dose supplementation.

As for heart "weakness" (which can refer to heart failure or general poor cardiac function), evidence supporting the efficacy of vitamin E is limited and inconsistent. Clinical guidelines do not recommend vitamin E supplements for the treatment or prevention of heart failure or other forms of heart "weakness." Overall, while the use of vitamin E for heart health has a scientific basis, the strength of evidence supporting its effectiveness is weak, resulting in a low evidence rating.
