Ligustilides are phthalide compounds primarily found in Ligusticum chuanxiong (Szechuan lovage) and Angelica sinensis (Dong quai), two traditional Chinese medicinal herbs. Historically, these plants have been widely used in Traditional Chinese Medicine (TCM) to treat "headache" conditions, including those attributed to sinus issues. In TCM, Ligusticum chuanxiong is often used in formulas such as Chuanxiong Cha Tiao San, which is prescribed for headaches caused by wind and dampness (including what modern medicine would call sinus headaches). The rationale is based on the herb's reputed ability to promote blood circulation, dispel wind, and relieve pain. Ligustilides are believed to contribute to these effects.

While there is some laboratory research indicating that ligustilides have anti-inflammatory, vasodilatory, and analgesic properties, there is a lack of direct clinical evidence validating the efficacy of ligustilides (or their source herbs) specifically for sinus headaches.
