Vitamin B6 (pyridoxine) is sometimes suggested as a supportive supplement in Hashimoto’s Disease, primarily due to its general role in immune function and thyroid hormone metabolism. However, the direct scientific evidence supporting its efficacy in treating or significantly improving Hashimoto’s thyroiditis is minimal. Some studies indicate that B6 deficiency may be more common in people with autoimmune diseases, including Hashimoto’s, but these findings are observational and do not establish causality or therapeutic benefit from supplementation.

Vitamin B6 is required as a cofactor for enzymes involved in amino acid metabolism and the synthesis of neurotransmitters, and it is also necessary for the conversion of the inactive thyroid hormone T4 to the active hormone T3. Theoretical rationale for its use stems from these biochemical roles. However, there are no robust clinical trials demonstrating that supplementing B6 in individuals with adequate levels improves thyroid function or alters the course of Hashimoto's Disease.

Most recommendations for B6 in Hashimoto’s are extrapolated from limited data on micronutrient deficiencies in hypothyroid patients. In summary, while there is a scientific rationale for avoiding B6 deficiency in Hashimoto’s, evidence supporting supplementation beyond correcting a deficiency is weak, and routine use is not broadly endorsed by clinical guidelines.
